Partnership strengthens care transitions and expands access to high-quality skilled nursing and rehabilitation services for patients across Connecticut
HARTFORD, Conn. - s4story -- National Health Care Associates, Inc. (NHCA) is proud to announce that three of its Connecticut skilled nursing and rehabilitation centers have been selected to join UConn Health's Preferred Post-Acute Care Network, strengthening access to high-quality post-acute care for patients across the region.

"We are honored that Maple View, Hebrew Center, and Pines at Bristol have been selected to join UConn Health Preferred Post-Acute Care Network," said Michelle Costa, Chief Clinical Officer, National Health Care Associates. "This recognition reflects the dedication, expertise, and unwavering commitment of our clinical teams to delivering exceptional patient care. Through this partnership, we look forward to strengthening care coordination, supporting seamless transitions from hospital to post-acute care, and helping patients achieve the best possible outcomes on their recovery journey."
As members of the Preferred Post-Acute Care Network, the three NHCA centers will collaborate closely with UConn Health providers to facilitate smooth transitions from the hospital, support individualized care planning, and ensure continuity of care throughout the rehabilitation process.
"Strong communication and coordination are essential when patients transition from the hospital to post-acute care. These centers have demonstrated the quality, collaboration, and patient-centered approach we value in our Preferred Post-Acute Care Network.
