Rethinking Standard Care

While topical corticosteroids remain first-line treatment, Dr. Caplan notes many women continue to experience persistent symptoms or lifelong steroid dependence despite guideline-directed care. She argues LS should be treated as an immune-mediated disease, noting that roughly one-third to one-half of affected women show autoimmune serological abnormalities, with links to thyroid disease, vitiligo, and type 1 diabetes.
