Transition provides medically supervised obesity care through a multidisciplinary team that includes Physician Associates, Nurse Practitioners, a Registered Dietitian, a physician Medical Director, behavioral health support, and fitness expertise. Services include medical evaluation, nutrition counseling, body composition analysis, fitness guidance, laboratory testing, behavioral support, structured meal options, and FDA-approved anti-obesity medications when appropriate. The practice also assists patients with prior authorizations for GLP-1 and other obesity medications.
