CMS Expands ACCESS Model to More Chronic Conditions, Broadening the Role of Technology-Supported Care
S For Story/10706250
s4story -- The Centers for Medicare & Medicaid Services (CMS) is expanding the Advancing Chronic Care with Effective, Scalable Solutions (ACCESS) Model to cover more chronic conditions. Beginning in spring 2027, the model will add new tracks for heart failure, chronic obstructive pulmonary disease (COPD), substance use disorders, and nicotine dependence, while extending support for certain chronic musculoskeletal conditions.
Expanding Technology-Supported Chronic Care
The expansion broadens the range of conditions that can be addressed through the ACCESS Model, which enables participating healthcare providers to provide technology-supported care between regular doctor visits. Depending on the care scenario, approaches can include virtual care, health coaching, remote monitoring, and connected devices or wearables. The model uses an outcomes-based payment approach that ties payments to specified health outcomes rather than individual services.
For healthcare providers and digital health programs, the broader range of chronic conditions highlights the need for flexible approaches to remote patient monitoring. Different conditions may require different types of measurements and monitoring, making connected medical devices an important component of technology-supported care workflows.

Connected Devices Across More Care Scenarios
As the ACCESS Model expands, connected devices can support different measurement needs across remote patient monitoring programs. Blood pressure and blood glucose measurements can support hypertension and diabetes management, while weight and SpO₂ measurements can be relevant to care scenarios involving cardiovascular or respiratory conditions.
Connectivity needs can vary across rpm programs and deployment environments. Transtek's 4G-connected devices use built-in cellular connectivity to support automatic measurement transmission without requiring smartphone pairing or home Wi-Fi configuration. Three-network connectivity can provide additional flexibility across different network conditions and locations, supporting deployment across diverse home monitoring environments.
For programs that need to connect multiple device types within a single home monitoring setup, Transtek also offers Bluetooth-connected devices that work with AnyHub. The AnyHub collects measurements from connected devices and uses built-in 4G connectivity to transmit them to Device Management Dashboards, providing another approach for multi-device remote monitoring workflows.
